Before your child’s first visit with ENT Care for Kids, please complete your new patient registration forms and follow the checklist so we can make the most of your valuable time with us.
You may download all of the forms at once, in the “new patient packet.” Or you can choose one of the individual forms. You can print a form and fill it out by hand, or fill it out on-screen before printing. You may also save a filled out form to your hard drive.
Please bring them to your appointment. For security reasons, we do not recommend sending these forms as email attachments.
